Chronic Disease Management


AMPATH, in partnership with the Government of Kenya, provides hypertension treatment and awareness programs in western rural Kenya. The LARK (Linkage and Retention to Care in Western Kenya) project addresses the challenges of linking hypertensive patients to clinical care and retaining those patient. Utilizing AMRS, the LARK project seeks to identify facilitators and barriers to patient linkage and retention. Through LARK, Community Health Workers (CHWs) will use mobile technologies and AMRS to reach patients and collect data.
